Although membership is free if you would like to make a donation to help with the running of the Patient Group that would be most welcome. You can donate by GoCardless, Standing Order, cheque or via the Charities Aid Foundation.
The links on the right take you to a GoCardless payment system where you will be requested to submit your bank details. All details are held securely byGoCardless and Friends of Davenport of Davenport House do not retain any bank details. Both you and Friends will receive a receipt of the set-up and we hope you will complete the Gift Aid declaration in order for the Charity to benefit from the tax relief.
If you would like the Friends of Davenport House to set up a standing order for you, you can download the form as shown below.

There are several ways to return these forms to us:
Mail it to, Friends of Davenport House, c/o Davenport House Surgery, Bowers Way, Harpenden, AL5 4HX
Drop it into the black post box, marked “Patient Group” on the wall to the left of the reception desk in the surgery.
Give it to the Surgery Receptionist
Scan it and send to website@friendsdavenporthouse.org.uk if you have completed the Gift Aid and Membership application only, if you have completed the Standing Orders part then we need the original so the document cannot be scanned.
If you are donating and pay UK tax the Friends of Davenport House can claim Gift Aid if you complete a Gift Aid declaration.
You can complete your Gift Aid Declaration online by clicking here.
Please notify the Treasurer if you change your name or address while the Gift aid declaration is in force. You can cancel the declaration at any time by notifying the Friends of Davenport House. It will then not apply to donations you make on or after the date of the cancellation or such dates as you specify.
